FireMed

All FireMed applications will be going through Life Flight now. You can pick up an application in our office, download the PDF below, or go to Life Flight's website by clicking here: member.lifeflight.org. The new applications have the option to sign up for either FireMed, Life Flight, or both. There will no longer be an application deadline; enrollment will be year round, however new members will have a 72 hour waiting period before coverage begins.

Please take time to read the FireMed Terms of Agreement before submitting an application or payment. Not everyone qualifies for FireMed Membership.

FireMed works in addition to your primary health, worker's compensation, and auto insurance, not as a replacement of.

One payment of $65 covers your household. In the case of multi-family households, each family will need to purchase FireMed separately even though they live at the same address. Membership includes coverage for yourself, your spouse or domestic partner and dependents you claim on your income tax return. Elderly and disabled family members living in the same household may also be covered.

You may purchase FireMed at any time.

Have an ambulance bill due? To ensure accurate application of your payment to your ambulance bill, please submit the ambulance bill information form to our office in addition to submitting payment.
